New Patient Registration
We welcome new patients. In order to register please either click on the link below or call into the surgery anytime between 9am and 6pm We ask that you avoid coming on a Monday as this is our busiest day. Please attach a form of photographic evidence and a utility bill to the online form. We also need to know the name and address of your previous GP.

Please note: Your registration will not be complete until you visit the surgery and sign the printed form. If you haven't filled out our online form you will need to bring a form of photographic ID and a utility bill showing your current address when you come in to the surgery to sign the form. You will then be given an information pack about the surgery and how to access our services.
Once you have registered we will book you an appointment for a new patient medical if you require one. If you are on any medication we will book you an appointment to see one of our Doctors.
If you live in one of the villages surrounding Brackley (apart from Middleton Cheney) you will be able to use our Dispensary to obtain both repeat and acute medication.
From 1st April 2015, the NHS has said that all practices are required to allocate a named accountable GP to all patients.
Our patients are already assigned a named GP, and you can find out who this is at your next interaction with the practice.
There will be no impact on the service you receive regarding the provision of appointments at Springfield Surgery, your treatment, or which GP you can see.
The name of your allocated GP will not necessarily be the one you see most regularly.
Please be assured that you can still access all of our medical team in exactly the same way as before.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
